As a Counselor, you will assist in diagnosing and creating a treatment plan for all clients. You will be a critical part of the client’s treatment program and assure they adhere to the guidelines of the facility’s programming. You will also keep detailed documentation of the client’s progress.

Requirements:

Associate degree in a related field
CAC/CADC certified or hold a current LPC (Licensed Professional Counselor) license.
COVID –19 Vaccination / approved exemption status

Responsibilities:

Screen patient for appropriateness of MMT (Methadone Maintenance Treatment) per State and Federal criteria for admission.
Explain and have patients sign all needed consent forms upon admission. Clearly explain the concept and exceptions of confidentiality.
Provide information about MMT and alternative treatments to ensure that the patient can give informed consent.
Review program rules and regulations with patients.
Complete psychosocial and treatment plan appropriate to patient’s condition.
Provide HIV education and perform risk evaluation. Record detailed intake note.
Schedule and meet with patients for individual or group counseling according to their individual treatment plan.
Maintain a caseload of up to 50 patients.
Record all counseling notes on the day that the session was held. Working with patients to develop individualized treatment plans and realistic goals.
Make referrals and follow-up on referrals as needed.
Complete treatment plans in the appropriate time frame (according to policy and procedure manual for every patient.
Complete and submit all patients’ medication related issues to the MD.
Enter and delete any Flags via MethaSoft computer program.
Make certain that patient records are always in the proper order and are stored in a confidential manner.
If a patient is discharged, complete discharge paperwork and submit these forms to the Medical Director for approval. Notify Medication Nurse and Receptionist of the discharge.
Notify nurses, Medical Director, and Clinical Director about any patient who may be suffering from a chronic illness (i.e., HIV/AIDS, TB, Diabetes, etc.).
Notify Medical Director, nurses, and Clinical Director of any patient who is pregnant.
Arrange guest dosing for patients: contact guest clinic; enter information into MethaSoft; print request; obtain the necessary signatures; fax request to guest clinic.
Understand all State and Federal laws and regulations.
Counselors must complete, at a minimum, 16 hours of training every year. This training may take place within the clinic or off site.
Conduct Group Therapy Sessions on a regular basis.  Participate in staff and treatment team meetings/ Participate in quarterly QI meetings.
